Application form for digital video player (“animal care” videos) loan service

The Agriculture, Fisheries and Conservation Department (AFCD) now provides lending services for digital video players to estates. The digital video players can display videos produced by this department. The videos are about animal care. They aim to educate the public about the dos and don’ts for handling dogs in public places and other knowledge on Responsible Pet Ownership so as to remind the public to treat animals well, take good care of their pets and keep their pets under proper control. If your estate is interested in applying for this service, you can fill in the e-form below.

Conditions of Loan

  1. Agriculture, Fisheries and Conservation Department provides loan of digital video players to Owners’ Corporation / Estates for free.
  2. The digital video players can be hung on the wall or put on the table (see photos). The digital video players need to be connected to a power supply. If the digital video players were damaged during loan, the damaged digital video players must be returned to this department. This department will not demand for compensation.

  3. Applications will be processed on a first come, first served basis and are subject to this department’s final approval. In the event that the requested digital video players are not ready for loan for any reason, this department may not be able to fulfil the request.
  4. If an application is successful, this department will confirm with the applicant by phone or fax so as to notify the applicant of the date and time for collection of the digital video players.
  5. The loan period for digital video players is three months. If an applicant / Owners’ Corporation / Estate would like to renew the loan of digital video players, please call 2729 6215 to apply for renewal at least three working days before the due date and during office hours. Applications for renewal will be processed on a first come, first served basis and are subject to this department’s final approval. In the event that the requested digital video players are not ready for renewal for any reason, this department may not be able to fulfil the request.
  6. Applicants / Owners’ Corporations / Estates should assign a representative to collect and return the digital video players on loan. When collecting the digital video players, the representative will be required to show his / her Hong Kong Identity Card or a valid staff card for verification of identity.
  7. Applicants are required to collect the digital video players on the assigned day and at the assigned time. If the digital video players are not collected at the assigned time, this will be deemed as voluntary waiver of the booking to loan the digital video players.
  8. The copyright of the videos displayed on the digital video players belongs solely to the Agriculture, Fisheries and Conservation Department.
  9. The on-loan digital video players shall not be used to display videos other than those pre-loaded by AFCD.
  10. In no circumstances may the applicant / Owners’ Corporation / Estate charge audiences for viewing the loaned items.
  11. The digital video players need to be connected to a power supply. The borrowing party needs to pay for the electricity bill himself / herself.
  12. This department is not liable for any accident, damage or harm caused to persons or fire caused while using the digital video players under the custody of the applicant / Owners’ Corporation / Estate.

Personal Information Collection Statement

  • The information provided by the applicant will be used by the Agriculture, Fisheries and Conservation Department for processing applications for loan of digital video players.
  • AFCD may give some of the information to other parties authorised by law to receive it.
  • In accordance with Section 18 and 22 and Principle 6 of Schedule 1 of the Personal Data (Privacy) Ordinance, applicants have the right to request correction of or access to the personal data contained in this application. The personal data contained in this form will be kept by this department for its own records.
